I used the New Testament bookmarks from lds.about.com and a google image of the TARDIS. Cropped the reading chart, pasted over the TARDIS doors, added a white box (for names). And there you go.
I think I'll put them all on a piece of poster board with a heading that reads:
"Scriptures - bigger on the inside".
